Challenge:
A 3-year-old, 13.5-kg boy with Philadelphia-chromosome positive acute lymphoblastic leukemia (ALL) was admitted with fever and pancytopenia.
Results:
The patient was continued on voriconazol and the dose was increased to 11 mg/kg over several days.
